Find out morePre-admission is an important part of your day hospital care. Your specialist will explain your procedure and complete the consent form with you during your consultation. If you have any questions about your planned day procedure admission, the completion of forms, costs or health insurance status, please contact our team who will be happy to assist you.
To ensure we can confirm your admission, health insurance and other details/arrangements, we ask that you please complete either the online pre-admission form, or paper admission form at least 5 days before your admission.
If you would prefer not to complete the online form, you can send us the paper forms usually provided to you by your surgeon. If you don’t have the paper forms please contact us before your admission date.
Please notify us if your weight is over 150kg prior to your admission as we will need to organise a pre-admission assessment.
Prior to admission be sure to confirm your level of cover for hospital admission with your health insurance provider. Here are some questions to ask
If your procedure is subject to Work Cover or a Third Party Claim, please send full details of the claim to the hospital, including a letter from your insurance company accepting liability for your admission to hospital for the procedure.
After a general anaesthetic, for your safety, you must have an escort to accompany you home, and a responsible adult must stay with you overnight. If these cannot be organised, please contact the day hospital at least two days prior to your admission to discuss your discharge plan with one of our registered nurses. We can assess your scenario and liaise with your specialist/anaesthetist and they can assist with discharge planning. If you do not have an escort, or overnight carer, then your procedure may be cancelled.
For children under 10, two responsible adults must be present in the vehicle, a driver and a carer.
If you can’t arrange an escort please contact our admissions staff who can put you in touch with a nursing agency. The agency can arrange for someone to act as your escort. This will incur a charge payable by you directly to them.
If you can’t speak or understand English well, and do not have someone who can speak English to accompany you to the hospital, then please ring the NSW Health Care Interpreter Service.

The service is free but you must arrange it before your admission day.
The hospital fee for your procedure covers the use of the operating theatre, day-stay accommodation and commonly used drugs and dressings. For some procedures, there will also be a charge for prostheses or disposables.
If you have private health insurance we will claim directly from your health fund.
If you have an excess or gap with your cover you will need to pay this on admission. Your health fund will have advised us directly of the cover they will pay.
If you do not have private health insurance you will need pay the full account on admission.
When we contact you, we will advise you of the hospital costs for your procedure, known as your Informed Financial Consent.
We accept cash, Visa and MasterCard, although do not accept AMEX or personal cheques.
Please be aware that your Specialist, Anaesthetist and Pathologist will send their accounts for their services separately and directly to you.
The Hospital does not handle these accounts and you will need to pay them directly.
One or two days prior to admission, admissions staff will contact you via SMS or phone you (if you have not given a mobile number) to confirm:
